At Charles E. Brown, III, DDS, PLLC we make every effort to provide you with the finest care and the most convenient financial options. To accomplish this we work hand-in-hand with you to maximize your insurance reimbursement for covered procedures. We participate in the following dental PPO networks: Delta, Guardian, Cigna, Aetna and Sun Life Assurant. Otherwise, payment for services will be based on out-of-network benefits. Your copayment is based on information provided by your dental benefit provider and our experience and knowledge with their policies.

Please be advised the benefits quoted are based on information obtained at the time of treatment and are not a guarantee of payment. Your account is due in-full within 30 days regardless of pending insurance.

Please provide our office with accurate dental insurance benefit information prior to your treatment so that we can expedite reimbursement. This will prevent delayed payment or denial of your claim. We agree with you, the dental benefit contract is between you and your dental provider. Due to HIPAA guidelines and restrictions, please contact your human resource department or dental provider if you have questions regarding benefit guidelines or disputes.

Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage.
